RN SUPERVISOR - FRIENDSHIP COURT ASSISTED LIVING

 

SCOPE

The RN Supervisor for Assisted Living oversees the Assisted Living department by participating in the planning, implementing, evaluating & supervision of services for tenants while complying with all state & federal regulations. You serve as an advocate & resource for the department while meeting organizational goals. The RN Supervisor will serve as a team member to promote the optimal level of functioning and the dignity of each tenant in the assisted living facility. You are essential for providing person-centered tenant care and providing a ministry of gentle service!

**Per Minnesota Department of Human Services guidelines, must have successful passing of background screening. **

ACCOUNTABILITIES

Leadership/Management

  • Develop & maintain person-centered tenant care philosophy
  • Develop & maintain a functioning Assisted Living organizational structure.
  • Assign & delegate responsibility & authority within the department
  • Partake in the recruiting process including interviewing, hiring and onboarding of Assisted Living staff
  • Responsible for adhering & implementing workplace policies – issuing disciplinary action and/or terminations, performance reviews, workers compensation process, payroll processes, etc.
  • Oversee scheduling of Assisted Living employees to ensure appropriate level of staff to meet current tenant needs.
  • Oversee Assisted Living staff, ensuring proper policy/procedures are being followed
  • Update departmental policies & procedures to reflect best practices related to assisted living facilities to correlate with State of MN & Federal regulation/compliance requirements.
  • Coordinate Assisted Living with other services within & outside the facility as needed
  • Participate in team meetings.

Tenant Related

  • Participate in assessing the eligibility of prospective tenants for assisted living services.
  • Participate in tenant admission process including admission assessment, service agreement, medication and treatment orders, and plan of care.
  • Orient staff to tenant’s plan of care.
  • Assist in providing nursing services as appropriate to each tenant’s plan of care.
  • Assist in conducting supervisory visits within 5 days of initiation of services and at least every 90 days.
  • Review and sign Medication and Treatment records weekly; review and sign PRN and Held Medication Records weekly or at frequency specified in tenant plan of care.
  • Assist in communicating changes in tenant’s health status to physician.
  • Assist in obtaining physician’s orders as necessary for medication and treatment changes.            
  • Assist in renewing medication and treatment orders at least every 12 months.
  • Participate in the review and revision of each tenant’s evaluation and service plan at least annually or more frequently when there is a change in the tenant’s condition that requires a change in services.
  • Summon EMS as appropriate to tenant need; complete transfer form and follow up with phone call to facility for emergency transfers.
  • Participate in discharge planning and discharge process.
  • Provide complete and accurate documentation of services.

Training/Development

  • Keep current on latest developments by attending professional seminars, institutes & professional journals.
  • Initiate & conduct assisted living related meetings while documenting content of training. This may include severe weather policy procedures & other emergency procedures.

Other

  • Demonstrate excellent communication & positive working relationships with all stakeholders within and outside the organization. Examples include tenants & their family members, volunteers, visitors, external agencies, healthcare professionals, internal departments, etc.
  • Ensure compliance within the assisted living department, organizational policies, systems, regulations and demonstrate a strong commitment to St. Luke’s Lutheran Care Center’s mission of providing a ministry of gentle service and person-centered care.
  • As part of the overall team, you may be requested to assist in the support of other resident care activities
  • Promote positive image of St. Luke’s Lutheran Care Center by embracing person-centered care with a positive attitude.
  • Be available to staff for consultation for any areas of concern
  • Serve on Safety Committee by offering ideas & solutions
  • Provide support for Assisted Living facility regarding culinary services & licensing policy/procedure
  • Wear name tag during scheduled shift
  • Adhere to the Code of Conduct and Compliance, Vulnerable Adult Policy, and any other policies in addition to other job specific policies
  • Attend scheduled mandatory in-services

PHYSICAL & SAFETY REQUIREMENTS

While performing the duties of this job, you are regularly required to walk, handle & feel objects, reach with hands & arms, reach above arm level, communicate effectively & efficiently in person & on the phone. In addition, the physical requirements for this role include, but are not limited to:

  • Ability to occasionally lift up to 60 pounds
  • Ability to occasionally carry up to 35 pounds
  • Ability to frequently bend, squat, crouch, kneel, and push/pull up to 20 pounds
  • Vision abilities required include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus
  • Follow standard precautions & infection control practices
  • Report any work or non-work related injury immediately

EDUCATION, EXPERIENCE & SKILLS

  • High School Diploma or equivalent.
  • Current Minnesota Registered Nurse or Licensed Practical Nurse.
  • Current CPR Certification. 
  • Experience in assisted living or home health care nursing, preferred.
  • Prior supervisory experience, preferred. 
  • Ability to maintain confidential information & work with a diverse population.
  • Ability to read & comprehend verbal & written instructions.
  • Ability to operate a computer & complete required training within the training program.
  • Ability to use GOOD BODY MECHANICS.
